Rates: Rates range from $155-$185 per 50-minute session. For further information regarding rates, please call.

Insurance: Dr. Rhoades does not participate with any insurance companies or plans. Therefore, he is considered an OUT-OF-NETWORK provider. Clients who carry insurance and want to use OUT-OF-NETWORK benefits should remember that professional services are rendered and charged to the clients and not to the insurance companies. Upon the client’s request, Dr. Rhoades will provide you a copy of your receipt and/or statement, which you can then submit to your insurance company for reimbursement, if you so choose. Dr. Rhoades may be required to disclose/produce a mental health diagnosis, a treatment plan, dates of service, types of service, and possibly other PHI. Dr. Rhoades will ask you to sign a release of information. If you so instruct him, Dr. Rhoades will communicate only the minimum necessary information to the carrier. Lastly, it is the client's responsibility to verify the specifics of their coverage and ultimately the client’s responsibility for payment of services rendered.

Check your health insurance or employee benefit plan to see what services may be covered in full or in part by your plan. Some questions you may want to ask regarding your coverage are:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• How much does my insurance pay for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee: Reduced fee services are available on a limited basis.
